• After you finish filling up the information, mail this request to the state office who handles those birth records. Then wait for them to send your duplicate birth certificate.

Asking Electronically:

• Go to the website of this Health Department who handles your birth records for the place you were born.
• Write down all the information about you on the requesting form.  Then after that you submit it.
• Using your credit card, pay the fee for the duplicate birth certificate. Then wait a mail to be sent to you.
